[CSCIUndergrad] guidelines for on-line courses

Colleagues,
   I would be very grateful if you could send me documents or links 
describing
any policies for faculty teaching on line courses.
  For example, 
Can any faculty member teach an on-line course?
Do they need certification/training/or permission?
Do they have to inform anyone they are teaching on-line?
How are course evaluations done?
Do we have a standard definition of "hybrid" and "asynchronous" (George
Otte's SLOAN RFP defines "hybrid" as 50% non-classroom and "asynchronous"
as nearly 100% non-classroom.

Our Technology/Library Committee is considering these issues and I would
like to make a report.
